📋 Tanggung Jawab Pekerjaan

• Conduct regular home visits to assess and monitor the health and well-being of seniors.
• Develop and implement personalized care plans for each client, in collaboration with healthcare professionals.
• Provide companionship and social engagement activities to promote mental and emotional well-being.
• Coordinate with healthcare providers to ensure timely medical care and follow-up.
• Manage client records and schedules using Microsoft Office and CRM systems.
• Participate in team meetings and training sessions to stay updated on best practices and new programs.
• Promote the organization's values and mission through community outreach and engagement.
• Report on client progress and any concerns to the care manager.

🎓 Kualifikasi & Syarat

• Bachelor's degree in Social Work, Gerontology, or a related field.
• Minimum of 3 years of experience in community care or senior care services.
• Certification in First Aid and CPR is a plus.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Familiarity with Microsoft Office and CRM systems.
